The aim of the study was to assess the effectiveness and safety of tadalafil (Cialis) in the treatment of patients with pulmonary arterial hypertension (PAH), a condition in which the left ventricle is unable to pump blood to the lungs. We aimed to compare the drug effect between the two treatment groups.

Patients

Eligible patients were those who had:

  • a history of PAH or chronic idiopathic pulmonary disease
  • a history of PAH during life
  • a history of PAH in the last 12 months
  • a history of PAH in the presence of comorbidities, including heart failure, hypertension and diabetes
  • a history of PAH in the presence of other systemic conditions
  • a history of PAH in the presence of myasthenia gravis
  • a history of PAH in the presence of hypertension

Study design

We conducted a cross-sectional study. Patients were randomised to one of two treatment groups: tadalafil (Cialis) 30 or 60 mg (1 mg) daily with no treatment in the evening; or sildenafil (Viagra) 40 or 80 mg (1 mg) daily with no treatment in the evening. In the treatment group, tadalafil (Cialis) 30 mg (1 mg) daily, compared with sildenafil (Viagra) 40 mg (1 mg), were administered for the treatment of PAH.

Data collection

Patients were interviewed to collect information on the patient's history, physical and psychological symptoms and quality of life. Efficacy assessments included the International Index of Erectile Function (IIEF) scale, the Pulmonary Function Impairment Questionnaire (PRQ-24) and the Global Assessment Question (GAB).

Statistical analysis

SPSS version 16.0 (SPSS, Chicago, IL, USA) was used for the analysis. A two-factor structure was considered. Patients' answers to the questions on IIEF, GAB and PRQ-24 were entered into a two-factor analysis. The ANOVA was used to test the effect of tadalafil (Cialis) 30 mg (1 mg) daily with no treatment on global assessment symptoms, functional capacity and global outcome measures. The Chi-square test was used to compare the two treatment groups. The results of the ANOVA analysis were compared using an analysis of variance (ANOVA) with the level of statistical significance (α<0.05).

The mean difference (MD) in the IIEF, GAB and PRQ-24 scores was 2.8 (SD 1.3) and 2.7 (SD 1.6) points, respectively. There were no significant differences between the treatment groups in the PRQ-24 scores. There was a significant difference in GAB scores between the two treatment groups (p<0.05).

The mean difference in the PRQ-24 scores was -0.19 (SD 1.3) and -0.25 (SD 1.5) points for Cialis and sildenafil, respectively (p<0.05).

The mean difference in GAB scores between the two treatment groups was -0.24 (SD 1.6) and -0.33 (SD 1.7) points, respectively (p<0.05).

Conclusion

The study showed that tadalafil 30 mg (1 mg) daily with no treatment could be a safe treatment for patients with PAH. It is recommended to continue tadalafil therapy for at least 12 months in patients with PAH. The effect of the treatment was statistically significant in patients with PAH and in patients with other systemic conditions.
